Job summary

Heritage at Turner Park Health & Rehab in Grand Prairie, Texas, is seeking a full-time Speech Language Pathologist (SLP) to support our in-house rehab team and residents through person-centered care.

The SLP will evaluate swallowing and cognitive concerns, create individualized treatment plans, provide AAC, educate families, and collaborate with the care team to improve residents’ communication and swallowing abilities. Active Texas licensure required.

Qualifications

• Active Texas license in Speech-Language Pathology.
• SNF/Long term care experience preferred.
• Knowledge of Net Health Electronic Documentation System preferred.

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Help residents improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled speech and language therapy assessments and develop individualized treatment plans.
• Provide AAC systems for severe expressive or language disorders.
• Train clients, families, and other professionals as subject matter experts.
• Coordinate with other professionals for diagnostic swallowing procedures as ordered by physician.
