Job summary

Evolve Therapy Services, LLC seeks a licensed Speech Language Pathologist to support an in-house Skilled Nursing Facility in Ponca City, OK.

This part-time or PRN role focuses on planning, implementing, and directing speech-language services with emphasis on documentation, FEES/MBS protocols, and collaborative care with residents, families, and staff.

Candidates must have a Master's in Speech Language Pathology, CCC, and an active license; ongoing education is encouraged.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency
