Job summary

Evolve Therapy Services, LLC seeks a compassionate Speech Language Pathologist for an in-house Skilled Nursing Facility in Midwest City, OK. This PRN role focuses on delivering patient-centered speech-language pathology services.

You will assess, treat, and document progress, collaborate with the care team, and ensure CMS-compliant documentation within NetHealth EMR. A Master’s degree, CCC, and an active license are required, with opportunities to grow within the organization.

Qualifications

• Master's degree in Speech-Language Pathology required.
• CCC (Certificate of Clinical Competency) required.
• Active SLP license in state(s) of practice.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Instruct patients, families, and staff in speech-language procedures for daily carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
