Job summary

Evolve Therapy Services is seeking a licensed Physical Therapy Assistant to join our in-house Skilled Nursing Facility program in North Platte, NE. This full-time opportunity (also offering part-time or PRN) supports a comprehensive therapy services team and emphasizes high-quality resident care under supervision of a Physical Therapist.

Key duties include resident screenings, treatment planning, individual and group therapy, and thorough CMS-compliant documentation in NetHealth EMR.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under PT direction.
• Assist in identifying targets and treatment plans with short-term and long-term goals.
• Conduct individual and group treatment including strength, mobility, gait, ROM, modalities.
• Monitor resident response and modify as indicated.
• Maintain resident schedules with PT and team members.
• Complete CMS-compliant documentation in NetHealth EMR.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license in state(s) of practice

Ongoing continuing education to maintain licensure and certification
