Evolve Therapy Services, a therapy management organization, seeks a compassionate Physical Therapist for our partner in Antioch, TN, to provide in-house SNF care. You will plan, implement, and supervise physical therapy programs in collaboration with an interdisciplinary team.

Responsibilities include screenings, evaluations, treatment planning, and compliant CMS documentation in NetHealth EMR. Active TN PT license and CEU maintenance required. Full-time, part-time, or PRN positions available.

Formación

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• CMS/EMR documentation requirements in NetHealth EMR.

Responsabilidades

• Provide physical therapy services to residents per established standards and productivity guidelines.
• Document and bill for physical therapy services according to procedures.
• Evaluate residents to determine treatment plans and goals.
• Develop treatment plans with short- and long-term objectives.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the care team.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.
