Job summary

Evolve Therapy Services is seeking a licensed Physical Therapist to join an in-house Skilled Nursing Facility in Antioch, TN. This role offers Full Time, Part Time, or PRN options to provide high-quality resident care and therapy services in accordance with standards and CMS expectations.

The ideal candidate has a BS in Physical Therapy, an active state license, and a commitment to ongoing professional development.

Qualifications

• BS in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Adhere to documentation and billing procedures for PT services.
• Demonstrate sound judgment in evaluation, planning, and follow-up of therapy programs.
• Screen residents regularly to determine intervention needs.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Interpret and communicate results to residents, families, and the care team.
• Complete CMS-compliant documentation in NetHealth EMR and perform other assigned duties.
