Job summary

Southwest LTC is seeking a Full-Time Speech Language Pathologist (SLP) to join our in-house team serving residents with person-centered care to achieve positive outcomes.

Responsibilities include evaluating swallowing and cognitive deficits, developing individualized treatment plans, and educating residents, families, and staff. TN license required with SNF/long-term care experience preferred; strong documentation skills are needed.

Qualifications

• Active Texas license in Speech-Language Pathology
• In good standing with regulatory agencies and licensing boards
• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different personalities

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Assist residents in goals to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled, medically necessary SLP assessments and develop individualized treatment plans.
• Implement AAC systems for severe expressive/language disorders.
• Provide training to clients, family, caregivers and other professionals.
• Coordinate with other professionals for swallowing procedures as ordered by physician.
• Demonstrate competent clinical skills and communicate changes in resident condition.
