Job summary

Evolve Therapy Services, LLC is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Worthington, MN. The role emphasizes delivering quality speech therapy, conducting evaluations, and planning treatment within CMS standards.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency required.
• Active SLP license in applicable state(s) required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate per standards and productivity.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target list, short- and long-term goals.
• Instruct patient, family, and staff to ensure carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to meet resident needs timely and with clinical excellence.
