Evolve Therapy Services is seeking a Speech Language Pathologist to provide in-house therapy services at a Skilled Nursing Facility in Duluth, MN. This position supports residents with speech-language pathology needs and collaborates with care teams to ensure high-quality care.

The role offers full-time, part-time, or PRN opportunities. Candidates must hold a Master's in Speech Language Pathology, CCC, and an active SLP license in the state, with ongoing continuing education to maintain

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsabilités

• Provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan that includes a target list, short-term, and long-term goals.
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities.
• Complete CMS-compliant documentation from CMS expectations in NetHealth EMR systems.
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs.
• Participate in screening protocols for establishing needs of residents timely and with clinical excellence.
