Job summary

Evolve Therapy Services in Columbus, NE is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility. The role involves planning, directing, and delivering speech-language pathology services to residents in accordance with applicable standards and regulations.

Responsibilities include FEES/MBS oversight, data-driven treatment planning, CMS-compliant documentation in NetHealth EMR, and collaboration with staff to meet resident goals.

Qualifications

• Master's degree in Speech Language Pathology is required.
• CCC (Certificate of Clinical Competency) is required.
• Active SLP license in state(s) of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with target lists and short-/long-term goals.
• Instruct patient, family, and staff in speech-language procedures for daily carryover.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screenings to ensure needs are met timely and with clinical excellence.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency (CCC)

Active SLP license in state(s) of practice
