Evolve Therapy Services in Columbus, NE, is seeking a Speech-Language Pathologist for an in-house Skilled Nursing Facility. The role focuses on assessing residents, planning treatment, and directing SLP services to maintain high standards of care.

You will document in CMS-aligned NetHealth EMR, collaborate with staff and families, and support FEES/MBS protocols when appropriate. Requires Master's in SLP, CCC, and an active state license; commitment to continuing education and professional growth.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency is required.
• Active SLP license in state(s) of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and short-/long-term goals.
• Instruct patient, family, and staff to ensure carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to identify residents' needs with clinical excellence.
