Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Beloit, WI. This full-time/part-time/PRN role supports residents with therapeutic services and requires collaboration with care teams to ensure quality outcomes.

The position emphasizes compliance with CMS expectations, documentation in NetHealth EMR, and ongoing professional development within a supportive organization.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency required.
• Active SLP license in state(s) of practice required.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop a treatment plan with target lists, short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols with site program coordination.
• Participate in screening protocols to meet resident needs timely and with clinical excellence.
