Job summary

Evolve Therapy Services in Burlington, WI seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility. This full-time (sign-on bonus) or PRN position focuses on planning, directing, and delivering speech-language pathology services to ensure high-quality resident care.

You will adhere to CMS guidelines and utilize NetHealth EMR for documentation, with opportunities for growth within the organization.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency (CCC) is required.
• Active SLP license in state(s) of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards and productivity.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target list, short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR.

Education
