Job summary

Evolve Therapy Services, LLC seeks a compassionate Speech Language Pathologist for an in-house Skilled Nursing Facility partner in Oconto, Wisconsin. This position is Part Time or PRN and supports high-quality resident care in a collaborative clinical setting.

The role involves evaluating residents, creating and implementing therapy plans, and documenting in NetHealth EMR to meet CMS requirements. Requires Master’s in SLP, CCC, and active Wisconsin licensure.

Qualifications

• Master's degree in Speech-Language Pathology.
• CCC certification or credential.
• Active SLP license in state(s) of practice.

Responsibilities

• Provide quality speech therapy services to residents.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target goals and timelines.
• Instruct patients, families, and staff in speech-language procedures.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Screen residents to identify needs and ensure timely care.
