Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility located in Webster, MA. Full Time, Part Time, or PRN positions are available.

The role focuses on planning, organizing, developing, and directing Physical Therapy services to ensure high-quality resident care. Responsibilities include evaluating residents, developing treatment plans, documenting in CMS-compliant systems, and collaborating with the interdisciplinary team to achieve goals.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice.
• Adhere to procedures regarding documentation and billing of physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents and families.
