Job summary

Fifth Star Care Management in Hampton, VA is seeking a dedicated Physical Therapist Assistant (PTA) to join our IN-HOUSE therapy team. New grads welcome and encouraged to apply, with a focus on resident mobility, strength, balance, and quality of life.

Your role includes implementing therapy plans under the PT, guiding exercises, and collaborating with nurses and other therapists to optimize daily functioning for residents.

Qualifications

• Associate degree from PTA program.
• Current state PTA license or certification.
• Experience in long-term care or geriatric settings preferred.
• Strong communication and organizational skills.

Responsibilities

• Implement PT treatment plans as directed by the Physical Therapist.
• Provide therapeutic interventions including exercises and gait training.
• Monitor progress and report changes to the supervising PT.
• Assist residents with mobility and daily activities.
• Educate residents and staff on safe mobility and use of equipment.
• Maintain accurate treatment documentation and records.
• Collaborate with nurses, OT, and SLP to support care plans.
• Adhere to facility policies and regulations.
