Job summary

Evolve Therapy Services is seeking a licensed Physical Therapist for our in-house Skilled Nursing Facility partner in North Platte, Nebraska. This full-time role offers salaried compensation and flexible scheduling.

You will provide therapy services, conduct screenings, and develop treatment plans while collaborating with the interdisciplinary team. A BS in Physical Therapy and an active license are required.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and company procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Collaborate with the interdisciplinary team and communicate evaluation findings and treatment plans.
• Develop treatment plans with short and long-term goals and implement them.
• Complete CMS-compliant documentation in NetHealth EMR systems.
