Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Nebraska. The role supports residents’ physical therapy needs and ensures high-quality care in collaboration with the interdisciplinary team.

Eligible candidates hold a BS in Physical Therapy, an active state license, and commit to ongoing licensure maintenance and professional development. Full-time, part-time, or PRN options are available with flexible hours.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Evaluate residents to develop and implement treatment plans with short and long-term goals.
• Conduct screenings to determine need for intervention or treatment and refer as needed.
• Communicate evaluation findings and plans to residents, families, and interdisciplinary team members.
• Complete compliant documentation in NetHealth EMR as required by CMS.
