Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and company procedures.
• Adhere to documentation and billing procedures.
• Develop and implement treatment plans with short and long-term goals.
• Evaluate residents to determine intervention needs and plan accordingly.
• Communicate findings and recommendations to residents, families, and the inter‑disciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
