Job summary

Crystal Lake Healthcare & Rehabilitation in Bayville, NJ is looking to hire a FULL TIME Physical Therapist Assistant to join our dedicated team at our in-house Skilled Nursing Facility.

Under the supervision of a Physical Therapist, you'll provide therapy services, help develop goals and treatment plans, monitor progress, document in NetHealth EMR, and educate residents and families while coordinating with the care team.

Qualifications

• Associates Degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Assist in developing target goals and treatment plans and scheduling.
• Monitor residents' response to intervention and modify treatment as indicated.
• Educate residents and families; document care in NetHealth EMR per CMS expectations.
• Collaborate with the rehabilitation team to ensure high-quality resident care.
