Job summary

Crystal Lake Healthcare & Rehabilitation in Bayville, NJ is seeking a compassionate Physical Therapist to join our rehabilitation team. This role offers Full-Time or PRN opportunities to evaluate and treat residents, coordinating with nurses, physicians, and other staff to implement individualized treatment plans.

You will contribute to quality outcomes, maintain CMS documentation in NetHealth EMR, stay current with evidence-based practices, and participate in ongoing education.

Qualifications

• Associates degree or higher in Physical Therapy.
• Active New Jersey Physical Therapy license.
• Commitment to continuing education and professional development.

Responsibilities

• Conduct comprehensive evaluations to determine therapy needs.
• Develop and implement individualized treatment plans.
• Provide therapy interventions to improve mobility, function, and independence.
• Educate residents and families on exercises and treatment plans.
• Document progress and adjust therapy plans as needed.
• Collaborate with healthcare team for care coordination and discharge planning.
• Ensure CMS and facility documentation compliance in NetHealth EMR.
• Maintain knowledge of evidence-based best practices in physical therapy.
