Evolve Therapy Services, LLC is hiring a Physical Therapist Assistant for an in-house Skilled Nursing Facility in Gatesville, NC. The role supports the Physical Therapist and the care team to deliver resident-centered therapy services.

Your responsibilities include screenings, progress documentation, and implementing treatment plans with a focus on mobility, strength, and balance. Licensure and ongoing education are required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Must have an active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsabilités

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and in developing short- and long-term treatment plans.
• Conduct individual and group treatments addressing strength, mobility, balance, and gait.
• Assist in selecting techniques/media and adjust activities per resident progress.
• Monitor resident progress and modify as directed by the Physical Therapist.
• Support scheduling and coordination with the team to maximize productivity.
• Train residents and families in use of equipment and care strategies.
• Document compliant and medically necessary CMS-compliant records in NetHealth.
• Participate in Quality Assurance activities.
