Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility located in Gatesville, NC. Multiple positions are available: full time, part time, or PRN.

The role focuses on planning, developing, and directing physical therapy services to ensure high-quality resident care. Responsibilities include evaluating residents, determining intervention needs, developing treatment plans, and documenting in NetHealth EMR.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents following established standards and productivity guidelines.
• Adhere to documentation and billing procedures for physical therapy services.
• Develop treatment plans with short- and long-term goals and monitor progress.
• Evaluate residents to determine need for intervention and plan accordingly.
• Complete CMS-compliant documentation in NetHealth EMR.
