Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility partner located in Gatesville, NC. The role supports Full Time, Part Time, or PRN positions with a focus on resident-centered therapy services.

You will plan, implement, and oversee physical therapy programs, conduct assessments, and coordinate with an interdisciplinary team to ensure compliance with CMS and licensure requirements.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards.
• Adhere to procedures regarding documentation and billing.
• Evaluate residents to determine treatment needs and plan.
• Develop treatment plans with short and long-term goals.
• Communicate findings to residents, families, and the care team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
