Job summary

Evolve Therapy Services is hiring compassionate Physical Therapists for an in-house Skilled Nursing Facility located in Gatesville, NC. Full Time, Part Time, or PRN positions available.

You will plan, organize, and direct Physical Therapy services in accordance with federal, state, and local standards, and collaborate with the Administrator to ensure high-quality resident care. A BS in PT and an active PT license are required.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate residents and develop short- and long-term treatment goals.
• Conduct regular screenings to determine need for intervention.
• Develop treatment plans and communicate findings to residents, families, and care teams.
• Complete CMS-compliant documentation in NetHealth EMR.
