Continuum Rehab Group is seeking a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care facilities in Albuquerque.

Licensed PTA with any experience level is welcome. You will provide skilled therapy services under the supervision of a Physical Therapist, document progress, and support discharge planning. The position offers a comprehensive benefits package and professional development opportunities.

Qualifications

• Licensed Physical Therapy Assistant required.
• All experience levels welcome.
• Evidence-based practice knowledge preferred.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides skilled therapy services under the supervision of a Physical Therapist.
• Assesses and documents patient progress and communicates with the supervising PT.
• Provides counseling to patients and families about physical disorders and rehab plans.
• Assists with discharge planning and treatment summaries as required.
