Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluation planning and treatment under the Physical Therapist.
• Develop target goals and treatment plans with the Physical Therapist.
• Deliver individual and group therapy including strength, mobility, balance and gait training.
• Monitor resident responses and modify interventions with therapist guidance.
• Coordinate resident schedules and document CMS-compliant records in EMR.

Education
