Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Gretna, NE. This PRN position focuses on delivering high-quality resident care and ensuring compliance with CMS requirements.

Responsibilities include evaluation, treatment planning, and documentation in NetHealth EMR, with collaboration across an interdisciplinary team. Minimum BS in PT and an active license are required, with ongoing continuing education encouraged.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and company procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of therapy programs.
• Conduct screenings of residents to determine need for intervention or treatment.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Interpret and communicate evaluation findings and treatment recommendations to residents and care teams.
• Complete CMS-compliant documentation in NetHealth EMR.
