Job summary

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents following standards of practice and company procedures.
• Document and bill therapy services according to CMS and department procedures.
• Perform evaluations to establish treatment plans and goals.
• Develop short- and long-term treatment plans and communicate findings to residents and families.
• Complete compliant documentation in NetHealth EMR as required.
