Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist to serve residents in an in-house Skilled Nursing Facility in Freeport, IL. This PRN role focuses on assessment, treatment planning, and resident-centered care with a collaborative interdisciplinary team.

You will perform evaluations, develop short- and long-term goals, document in NetHealth EMR, and ensure compliance with CMS standards while promoting quality resident outcomes.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per standards and productivity guidelines.
• Document and bill PT services per procedures.
• Evaluate residents to determine need for intervention/treatment.
• Develop short- and long-term treatment goals and methods to achieve them.
• Interpret and communicate evaluation findings and plans to residents, families, and team.
• Complete CMS-compliant documentation in NetHealth EMR.
