Evolve Therapy Services, LLC is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Oklahoma City. This full-time role includes premium pay and a sign-on bonus, with responsibilities spanning evaluation, treatment planning, and interdisciplinary collaboration.

You will ensure CMS-compliant documentation, maintain licensure, and contribute to high-quality resident care in a structured clinical setting.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.
• Ability to document per CMS expectations and standards.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and productivity expectations.
• Adhere to documentation and billing procedures.
• Demonstrate sound clinical judgment in evaluation, planning, and follow-up of therapy programs.
• Conduct regular resident screenings to determine need for intervention or treatment.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Communicate evaluation findings, plans, and recommendations to residents, families, and the interdisciplinary team.
