Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Saegertown, PA. This PRN role focuses on planning, directing and delivering physical therapy to residents, while ensuring CMS-compliant documentation and interdisciplinary collaboration.

The position requires a BS in Physical Therapy, an active license in the state, and commitment to ongoing professional development to keep licensure current.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Commitment to ongoing continuing education to maintain licensure.

Responsabilités

• Provide physical therapy services to residents per standards and procedures.
• Document and bill therapy services per CMS and CMS expectations in NetHealth.
• Evaluate residents and develop treatment plans with short/long-term goals.
• Screen residents regularly to determine need for intervention/treatment.
• Communicate findings and recommendations to residents, families, and team.
• Maintain compliance with regulatory standards and organizational policies.
