Evolve Therapy Services is recruiting a Physical Therapist for an in-house Skilled Nursing Facility in South Sioux City, NE. The role offers a $10,000 sign-on bonus and options for full-time, part-time, or PRN schedules.

You will plan, develop, and direct physical therapy services, ensuring CMS-compliant documentation and high-quality resident care in collaboration with the interdisciplinary team. Licensure, CE, and ongoing professional development are required.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screenings of residents to determine need for intervention/treatment.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
