Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Trotwood, OH. This PRN position requires licensure in the state and involves planning, directing OT services, and collaborating with an interdisciplinary team to ensure high-quality resident care.

Responsibilities include screening residents, developing treatment plans with goals, documenting progress, and supervising COTAs as needed.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents regularly to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans with goals.
• Interpret and communicate evaluation findings to residents, families, and the care team.
• Establish individual therapy schedules with team members.
• Conduct therapy per plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA and ensure documentation and treatments meet reimbursement practices.
