Evolve Therapy Services is seeking a licensed Physical Therapist to serve residents in an in-house Skilled Nursing Facility in Ohio. The role includes planning, implementing, and evaluating PT services while adhering to CMS and facility standards.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development and learning.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Document and bill PT services according to CMS and organizational guidelines.
• Evaluate and develop treatment plans with short- and long-term goals.
• Screen residents to determine need for intervention and adjust plans accordingly.
• Communicate findings and recommendations to residents, families, and interdisciplinary team.
• Complete compliant documentation in NetHealth EMR consistent with regulations.
