Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Rochester, MN. This Full Time or PRN role focuses on delivering high-quality resident care and meeting CMS and therapy standards.

The successful candidate will plan, implement, and assess therapy services, maintain compliant documentation in NetHealth EMR, and collaborate with the interdisciplinary team to maximize resident outcomes and satisfaction.

Qualifications

• Must hold a Bachelor of Science in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents at regular intervals to determine need for intervention.
• Conduct timely screening of residents referred to physical therapy to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents and the interdisciplinary team.
• Complete CMS-compliant, medically necessary documentation in NetHealth EMR.
