Job summary

Evolve Therapy Services in Blue Hill, NE is seeking a licensed Physical Therapist for our in-house Skilled Nursing Facility. This PRN position focuses on delivering high-quality therapeutic services to residents and supporting an interdisciplinary care team.

You will assess residents, develop treatment plans, document per CMS and NetHealth EMR standards, and participate in ongoing professional development to maintain licensure.

Qualifications

• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per standards and productivity guidelines.
• Document therapy services according to CMS expectations in NetHealth EMR.
• Demonstrate sound judgment in evaluation, planning, and follow-up of therapy programs.
• Screen residents to determine need for intervention or treatment.
• Evaluate residents to obtain data for planning and implementation.
• Develop short- and long-term goals and treatment plans.
• Communicate findings and plans to residents, families, and the interdisciplinary team.
• Complete compliant and medically necessary documentation.
• Perform additional duties to support resident care and organizational needs.
