Job summary

Evolve Therapy Services, LLC seeks a dedicated Physical Therapist for an in-house Skilled Nursing Facility in Oklahoma City. You will plan, direct, and provide physical therapy services to residents in line with standards and CMS expectations.

Responsibilities include evaluation, treatment planning, documentation, and interdisciplinary communication. Requires ACS or equivalent PT licensure and ongoing professional development to maintain licensure.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents following applicable standards and procedures.
• Document and bill therapies per CMS and Company procedures.
• Develop and implement treatment plans with short and long-term goals.
• Conduct screenings and evaluations to determine intervention needs.
• Communicate findings to residents, families, and the interdisciplinary team.
