Job summary

Apploi is seeking a Physical Therapist for an in-house Skilled Nursing Facility in a part-time or PRN capacity. You will provide PT services, develop treatment plans, and document in NetHealth EMR to CMS standards.

Requirements include a Bachelor of Science in Physical Therapy and an active PT license in the state of practice, with ongoing continuing education to maintain licensure. Growth opportunities may be available.

Qualifications

• Bachelor of Science Degree in Physical Therapy is required.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and company procedures.
• Document and bill per CMS and company procedures.
• Evaluate residents and develop treatment plans with short/long-term goals.
• Perform screenings to determine need for intervention/treatment.
• Communicate findings and recommendations to residents, families, and team.
• Complete compliant documentation in NetHealth EMR as required.
