Job summary

Apploi is seeking a part-time or PRN Physical Therapist to provide in-house skilled nursing facility services. The role focuses on planning, organizing, and directing PT care aligned with federal and state standards to ensure high-quality resident outcomes.

Responsibilities include evaluations, treatment planning, and interdisciplinary communication, with emphasis on compliant documentation in NetHealth EMR. Requires licensure and continuing education. Part-time or PRN status.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Conduct evaluations to determine need for intervention and develop treatment plans with short- and long-term goals.
• Communicate evaluation findings and recommendations to residents, families and interdisciplinary team.
• Complete compliant CMS-mandated documentation in NetHealth EMR.
