Job summary

Evolve Therapy Services, located in Prince Frederick, MD, is seeking a full-time Physical Therapist for an in-house Skilled Nursing Facility. The role focuses on planning, directing, and providing physical therapy services to residents in accordance with federal, state, and local standards.

Responsibilities include evaluating residents, developing treatment plans, documenting in NetHealth, and collaborating with an interdisciplinary team to ensure high-quality resident care.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and productivity requirements.
• Adhere to procedures regarding documentation and billing of physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screenings of residents to determine need for intervention/treatment.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Interpret and communicate evaluation findings, treatment plan and recommendations to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
