CareOne is seeking a Physical Therapist to join our in-house rehab team in East Longmeadow, MA. You will provide high-level resident care, work closely with nursing and clinical staff, and contribute to strong patient outcomes in a stable team environment.

Qualifications

• Master's or Doctorate Degree in Physical Therapy.
• Current and Active Physical Therapist license in the operating state.
• Experience with older adult rehabilitation is preferred.
• Knowledge of governmental and regulatory guidelines in healthcare settings is preferred.

Responsibilities

• Provides and directs resident rehabilitation care following standard practices.
• Evaluates residents and creates effective treatment plans.
• Coordinates supervision of treatment plans with staff and other disciplines.
• Performs clinical documentation to meet regulatory requirements.
• Maintains billing records to meet regulatory requirements.
