Job summary

Evolve Therapy Services is hiring an Occupational Therapy Assistant for an in-house Skilled Nursing Facility located in Ahoskie, NC. This full- or part-time/PRN opportunity focuses on providing practical, quality care under supervision to our residents with a strong emphasis on documentation and compliance.

The OTA will screen residents, develop and implement treatment plans, and communicate progress with the interdisciplinary team.

Qualifications

• OTA licensure active in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to identify best intervention and treatment per scope of practice.
• Implement treatment plans and progress toward goals per plan of care.
• Communicate evaluation findings, plans, and recommendations to residents, families, and team.
• Schedule resident therapy with input from team members.
• Document and bill occupational therapy services.
• Adhere to CMS documentation standards daily.
• Perform other duties as assigned to support patient care.
