Job summary

Evolve-Therapy-Services in Lehigh Acres, FL seeks an Occupational Therapist for an in-house Skilled Nursing Facility. This Full Time role also offers Part Time or PRN options, with a flexible schedule to suit staff availability.

You will screen residents, develop and execute individualized treatment plans, and collaborate with a full interdisciplinary team. Supervising a COTA and accurate documentation are essential.

Qualifications

• Degree in Occupational Therapy.
• Must have an Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention for each patient.
• Evaluate residents to inform treatment plans and establish goals.
• Communicate evaluation findings, plans, and recommendations to residents, families, and the team.
• Establish individual therapy schedules with other team members.
• Conduct therapy per plan and ensure proper documentation and billing.
• Oversee COTA as needed and ensure documentation and reimbursement practices are followed.
