Evolve Therapy Services, LLC is hiring a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Olivia, MN. You will plan, organize, and direct OT services to ensure consistent, high-quality resident care in compliance with current standards and regulations.

Qualifications

• Clinical assessment and evaluation of residents.
• Development of individualized treatment plans with goals.
• Participation in interdisciplinary care planning.
• Documentation of therapy interventions and progress.

Responsibilities

• Screen residents to identify the best OT intervention for each patient.
• Evaluate residents and develop short- and long-term treatment goals.
• Communicate findings and recommendations to residents, families, and team members.
• Establish resident therapy schedules with input from the care team.
• Deliver OT services per treatment plan and document/bill accordingly.
• Oversee COTA and ensure documentation and reimbursement compliance.
