Job summary

Evolve Therapy Services is hiring an Occupational Therapist for an in-house Skilled Nursing Facility. The role is a full-time position with sign-on bonus and tuition reimbursement, plus part-time or PRN options.

Responsibilities include planning and directing OT services, supervising COTA, coordinating with the interdisciplinary team, and ensuring proper documentation and billing to deliver high-quality resident care.

Qualifications

• Screen residents to identify best interventions.
• Evaluate residents to inform treatment plans and goals.
• Communicate findings and plans to residents, families, and team.
• Establish resident therapy schedules with the team.
• Document therapy and ensure proper billing and compliance.

Responsibilities

• Plan, organize, develop, and direct Occupational Therapy services in the facility.
• Oversee COTA and ensure treatments and documentation comply with standards.
• Provide therapy per approved treatment plans and adjust as needed.
