Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility partner located in Church Hill, TN. This PRN role focuses on delivering high-quality therapy to residents and collaborating with an interdisciplinary team.

You will screen residents, develop and implement treatment plans, coordinate with families and staff, document services, and supervise COTA as needed.

Qualifications

• OT degree and active license in applicable state.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Evaluate residents to inform and develop treatment plans and goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and interdisciplinary team.
• Establish individual resident therapy schedules with the team.
• Conduct therapy according to the treatment plan and document/bill services.
• Oversee COTA and ensure documentation and reimbursement practices meet standards.
