Evolve Therapy Services in New Haven, CT is hiring a dedicated Occupational Therapist for an in-house Skilled Nursing Facility. Qualifications

• Must hold an OT degree and an active Occupational Therapy license in the practicing state.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Conduct regular screenings to determine best intervention for each resident.
• Evaluate residents to inform treatment plans with short- and long-term goals.
• Communicate evaluation findings and plans to residents, families, and team members.
• Establish individual therapy schedules with the care team.
• Provide therapy per plan and ensure proper documentation and billing.
• Perform duties to treat patients and support clinical excellence.
• Oversee COTA and ensure documentation and treatments meet reimbursement standards.
