Job summary

Evolve Therapy Services is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in New Haven, CT. This full-time or PRN position focuses on planning, organizing, directing occupational therapy services to ensure the highest quality resident care.

Responsibilities include screening residents, evaluating and developing treatment plans, communicating findings to residents and families, coordinating with the interdisciplinary team, scheduling therapies, documenting and billing,

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to figure out the best intervention or treatment for each individual patient.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed.
