Job summary

Evolve Therapy Services is hiring an Occupational Therapist for our partner located in Olivia, MN, in an in-house Skilled Nursing Facility. This full-time role, with part-time or PRN options, offers a $5,000 sign-on bonus plus tuition reimbursement.

The OT will plan, organize, develop, and direct OT services in the facility, ensuring compliance with standards and high-quality resident care. Licensure in the practicing state is a must.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to identify best intervention or treatment.
• Evaluate residents to develop treatment plans and goals.
• Communicate findings and plans to residents, families, and team.
• Schedule resident therapy with team members.
• Document therapy sessions and ensure correct billing.
